JIU-JITSU SEMINAR LIABILITY WAIVER AND RELEASE
STATE OF COLORADO
This Jiu-Jitsu Seminar Liability Waiver and Release (the "Waiver") is executed by the undersigned participant (the "Participant") on the date set forth below in favor of Ghost Squad Castle Rock, its owners, instructors, employees, volunteers, representatives, agents, successors, and assigns (collectively, the "Released Parties").
1. ASSUMPTION OF RISK
I, the undersigned Participant, acknowledge that participation in the Jiu-Jitsu seminar involves inherent risks, including but not limited to, physical contact, falls, collisions, and strenuous physical exertion, which may result in serious injury, permanent disability, paralysis, or death. I voluntarily assume all risks associated with my participation in the seminar and acknowledge that I am solely responsible for my safety and well-being.
2. WAIVER AND RELEASE OF LIABILITY
I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, discharge, and hold harmless the Released Parties from any and all claims, liabilities, demands, damages, actions, or causes of action arising out of or related to any injury, disability, death, or property damage sustained in connection with my participation in the seminar, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. MEDICAL CONSENT AND FITNESS TO PARTICIPATE
I certify that I am physically fit and have no known medical conditions or impairments that would prevent my safe participation in the seminar. I consent to receive medical treatment in the event of injury, accident, or illness during the seminar and understand that I am responsible for any related medical expenses.
4. INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, losses, or damages, including attorney's fees and costs, arising from my participation in the seminar or any breach of this Waiver.
5. MEDIA RELEASE
I grant permission to the Released Parties to use photographs, videos, or other media taken during the seminar for promotional, marketing, and educational purposes without compensation.
6. GOVERNING LAW AND SEVERABILITY
This Waiver shall be governed by and construed in accordance with the laws of the State of Colorado. If any provision of this Waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
7. ACKNOWLEDGMENT OF UNDERSTANDING
I have carefully read this Waiver, fully understand its terms, and voluntarily sign it. I acknowledge that by signing this Waiver, I am waiving certain legal rights, including the right to sue.
